Here is what to expect, what to bring and where to fill in the forms ahead of time.
Our forms are on our patient portal, not on this website. Fill them in before you come, or arrive 15 minutes early and we will help.
What happens

Step 1: Book
Use the booking link or call. Ask for an early or a lunch-hour time if that suits your day.

Step 2: Forms
Fill in the patient forms ahead of time, or at the office.

Step 3: The exam
Measurements, an eye health check, and a talk with the doctor about how you use your eyes.

Step 4: Before you leave
A copy of your prescription, a written estimate for anything you pay yourself, and your next visit if you need one.
